We maintain procedural safeguards to protect such personal information, including using SSL encryption when you provide confidential personal information to us online, limiting access to and disclosure of documents that contain your confidential personal information, and when we are required to share your confidential personal information to provide you with services, requiring such providers to sign confidentiality agreements with respect to your information. We do not share your confidential personal information with third parties unless it is necessary for business purposes, such as providing membership benefits, or is required by law, or we obtain your consent. In the event of a sale, merger or similar transaction affecting Welby Health Association, your confidential personal information, including your Social Security number, may be transferred to the new company, subject to the restrictions and protections of this Welby Health Association privacy policy. Before we dispose of any hard or electronic copies of documents that contain your Social Security number or other confidential personal information, we shred, destroy or otherwise make unusable such copies.
